Lytle’s K-9’s - German Shepherd Dog/Puppy Purchase Request Form I
_______________________(Buyer) request to purchase a purebred German Shepherd Dog from Lytle’s K-9’s. I am wishing
to purchase a (Circle one) dog/bitch from the (A,B,C,etc.) _____ litter whelped or due to whelp on _____/_____/_____. Sire
of the litter is ____________________. Dam of this litter is ____________________. I am wanting to purchase this puppy to
be used for (circle all that apply) sport, protection, family companion, sar, possible breeding stock, police service, other.

I (Buyer) am
sending a deposit in the amount of $300 USD to hold a puppy in reserve and fully intend to purchase the puppy by sending the
remaining balance of $__________ USD by the time the puppy reaches 6 weeks of age. I understand that this deposit is NON-REFUNDABLE
and may have to be applied toward another puppy from another litter if no puppy is available for me from the litter described
above. I also understand that I will have to pay an additional amount of $300 USD if I need to have the puppy shipped. I understand
this additional amount will cover the cost of a crate, veterinarian health certificate, transportation to Anchorage International
Airport and the actual shipping expenses via the airline that best serves me.
I have read and understand the health
guarantee that comes with this puppy. I have printed and signed a copy that is included with this purchase request form. In
doing so, I understand that Lytle’s K-9’s will sign and deliver to me the same guarantee with the puppy packet
that will be mailed to me at the time I take possession of the puppy described above.
I enter into this agreement in
good faith. I will abide by the guarantee and the actual purchase agreement to be sent to me once Lytle’s K-9’s
reviews my purchase request. I understand that I will only be sent this purchase agreement once Lytle’s K-9’s
reviews this purchase request form and approves me as a buyer. I understand the only way I will receive my deposit back is
if Lytle’s K-9’s is not willing to sell me a puppy or if they cannot provide me with a puppy within the next 6
months from the date signed below. I also understand that Lytle’s K-9’s has the right to refuse me the purchase
of this puppy, any puppy, dog or service for ANY reason.
